Welcome to the Cleveland Clinic Health System Itemized Billing Website

In order to assist you, The Cleveland Clinic Health System has outsourced the provision of medical bills (Itemized and Insurance Billing Forms) when requested by attorneys, auto-insurers, and non-contracted payors.

The new process will enable you to receive the requested documents within a reasonable time frame, with emphasis on efficiency but more importantly, effectively. Click on the above Links for further details on each topic.

  1. If the patient needs a copy of their bills (Itemized – Not Insurance Billing Forms), the patient may contact Customer Service at 216-445-6249.

  2. If the patient needs to discuss a payoff amount or balance on their account, the patient may contact Customer Service at 216-445-6249.

  3. If the patient needs help paying their medical bills, the patient may contact Customer Service at 866-621-6385 or go to www.ccf.org/financialassistance for information on The Cleveland Clinic’s financial assistance policy.

  4. Requests are handled on a first come, first served basis and we do ask that you allow up to 30 days before expecting to receive your bills.

  5. There are certain circumstances where parties, including your client, are entitled to one free copy. (See O.R. C. § 3701.741(C)). Please advise us immediately if this is applicable to your client and a free copy will be provided upon verification of the exemption.
